Parkview Clinic Data Privacy Notice



Dear Patient,

As part of your treatment our therapists will need to collect certain data and information from you in order to carry out a safe and effective job.

Please read below about how and what information we need to collect, how we store it, who can access it and how we destroy it. You must sign and consent to this for us to be able to treat you.

What information we collect

As a business working in the health and medical sector we have a legal obligation to request certain “special category data” about you. This includes details about your health, occupation, lifestyle & medical history that may have relevance to your presentation. In order to process and accurately record this we also need to record profile data such as name, age, address and contact information.

What we do with your data

All of the information mentioned above is used by our therapists in the evaluation of your symptoms for diagnostic purpose and recording of your case history. Occasionally some data may be used for auditing and research purposes, but this would be anonymised first. In some cases, we may share your information with other medical practitioners, health insurance companies or in legal proceedings, however, we do not share your data without your expressed consent. No data is ever sold on to other businesses.

How we store your data

Medical records are considered a legal document and we are obliged to store them for 8 years from the date you were last seen in clinic. For children this is until their 25th birthday. The majority of your data is kept on paper based records in locked metal cabinets or in secure archive. Some data is stored on the business’s PC hard drives and cloud back ups all of which are password protected and only accessible to our people. For communication we use online mailing services diary management software who we have checked for their GDPR compliance.

Your rights of access

You have the right to be informed about how we use your data. Should you wish to have copies of your data please just ask in writing, proof of ID will be required, but no charge is applicable. Should you have any complaints about the way we handle your data please ask for a copy of our complaints procedures and submit to us accordingly. We take the control of your data seriously and do our utmost to ensure none can be lost, stolen or damaged. After the term of our storage obligation has expired, your data will be incinerated or deleted permanently. We are registered with the ICO.

When you arrive for treatment you will be asked to read and sign this notice to acknowledge that you have read our Data Privacy Notice and that you consent for us to collect, use and store this data. For patients under the age of 18 parents consent is also required.
